Determination is made about what level of care and what type of facility would be most appropriate.

The client's financial situation is reviewed to determine what is affordable.

We will provide personal tours of facility options.

Support is provided to the family throughout their decision-making process.

Coordination is provided with Social Workers and RN Case Managers to arrange services and equipment for client's discharge and transfer to the chosen facility.

New Beginnings will arrange transportation for the day of discharge.

New Beginnings provides personal follow-up in homes and facilities after placement.
Every client's situation is different, requiring careful attention to individual needs. For this reason, all of our clients and family members receive personal, one on one guidance through the placement process.
In addition to providing high-quality placement service, New Beginnings strives to assist our clients with all other aspects of the transition as well.
